Lineup Cards

 

Lineup Cards Procedures

It is a requirement to have PRINTED lineup cards for all games and it is preferred that all coaches print their own lineup cards.  Below is a step by step how to.  Cards need to be printed on card stock paper not regular paper.

DOWNLOAD CARDS

•       Click Lineup Cards then click download to fill in information.

FILLING OUT CARDS

•      Fill in region, age group, and team number

•       Fill in your team’s name

•       Fill in coach’s name and assistant coach’s name

•       Fill in your entire team roster in numerical order, including first & last name as it appears on your official roster

PRINTING LINEUP CARD

•       Paper needed: Legal size card stock paper (8.5” x 14”)

•       Insert legal size card stock into your printer, be sure that you reset paper size

•       Change your settings

º Paper size: Legal

º Scale: Fits to paper

º Two-Sided: Print on both sides, flip on long edge

º  Click print, each sheet will provide 4 complete game cards

º  Print enough for the present season

º  Example of sheet once printed

º   Once printed cut cards equally in half in both directions

GAME DAY

COACH

Before the game:

•       Fill in date of game

•       Fill in your opponent’s name

•       Write "sick" or "absent" next to the player’s name if they are not in attendance

REFEREE

Before the game:

•       Referee Game Report on back side of card fill in game date, time, field number (if applicable), and conditions

•       Fill in home team name and main uniform color and opponent name and main uniform color

REFEREE

During the game:

•       Use tally marks to record goals scored by each player. Use one column for each half of the game

•       Record who is playing keeper with a "G"

•       Record substitutes i.e. those not playing with an "X"

•       Record the halftime score and who is winning

•       Record any cautions or send offs

REFEREE

After the game:

•       Write final score, winning team and losing team

•       PRINT your name (each referee) and include your team #

•       Record any disciplinary actions, significant injuries, or any additional comments in the Preliminary Incident Report section

•       If Preliminary Incident Report section is used referee and assistant referees must sign
